2. Fill it out!
Complete an account application form. Please make sure that all the pages are filled out completely, including all references & bank information available to Team Marketplace.
In addition to the application we will also need:
1. A signed current financial statement that includes a detailed balance sheet, profit and loss statement, and notes. If your business is less than one year old, please attach a start up balance sheet and a business plan with cash and sales projections for the first year of operation.
2. Please include a copy of your state tax exemption certificate with your application.
3. Send it in!

1. Your application

3. A copy of your state’s tax exemption certificate.
